We begin our services by coordinating a no obligation complete head to toe assessment by our Care Coordinator. In most cases we will communicate with the patient and family, discharge planner, social worker, case manager and primary
physician, to develop a care plan that is tailored to meet your
needs. Periodic visits by the Care Coordinator are arranged to
manage the care plan, allowing the family or Case Manager to
effectively monitor the progress of each patient, and to insure
that the best care is being provided at all times.
You are encouraged to meet your primary caregiver before the commencement of services. We do this for many reasons, most importantly to discuss the care plan and make sure you feel comfortable with the caregiver you will be working with. Our professional staff is completely insured and bonded, and has successfully passed a complete criminal background check. Our services can be arranged for a few hours a week or as many as 24 hours a day.
Long term care or short term respite relief is available seven days-a-week, including holidays. Attendant Care Services of Michigan provides helpful and necessary services that reduce the difficult, time consuming, and often stressful aspects of personal home care. We are there to assist you.
